Abstract
State Elementary School Pandean Lamper 05 Semarang is one of the schools that implement teacher performance evaluation process in order to ensure quality learning for all students and determine teachers promotion. That becomes the problem is the assessment process is very complicated, if done manually it will take a long time and vulnerable an errors on assessment report along with other data reports. This makes the assessment of teacher performance becomes ineffective. Based on the problems that occur, the authors make the Teacher Performance Assessment Information System in order to facilitate the data collection, data search, process assessment, and reporting. Research methodology is a field study included observations and interviews, as well as the study of literature. System development method used is Prototyping. This system uses the Java programming language and MySQL database GUI. From the analysis, it can be seen that the institution requires new systems to improve the effectiveness and efficiency of the teacher performance appraisal process.
